A student who is enrolling for the first time (undergraduate, graduate, exchange, visiting, NGA, etc.), who has recovered after expulsion or returned from academic leave (1 year or more), as well as applying for residence in the 69色 Dormitory, is required to fill out and submit to the Medical Service Office the following documents:

1. Medical form
2. Certificates, reports of medical specialists, test results specified in the

All documents are required to be submitted to the Medical Service Office within the following deadlines:

• for a student admitted for the first year of study - within 10 working days from the date of signing the 鈥淎greement on Educational Services鈥�;
• for a student returning from academic leave - on the day of application for readmission;
• or a student applying for residence in the 69色 Dormitory - on the date of application for a place to reside in the 69色 Dormitory.

Medical documents are submitted to the Medical Service Office in Kyrgyz, Russian, or English, only one copy, in one language version! 听In case the documents are registered in other languages, a notarized translation must be attached. Original medical reports must be certified by a legible stamp of the medical institution, stamp, and signature of the doctor.

Each year, in September, or within other deadlines established by 69色 order, all 69色 students must get fluorography arranged by 69色.听

In case an 69色 student misses fluorography arranged by 69色, he/she shall get fluorography on his/her own within 1 week and submit to the Medical Service Office a report/certificate by the relevant medical specialist with the results of the examination. 69色 has the right to apply administrative measures to those who violate this rule.
